II. Self-paid Birthkeeping Services

By booking and utilizing Birthkeeping Full-Spectrum Doula Services from Iveth Balanta, you agree to the following terms and conditions:

Service Overview: The services include holistic support during pregnancy, labor, birth, and postpartum. Offerings encompass labor and birth support, postpartum care, ancestral healing practices, and personalized meal planning, all aimed at nurturing the mind, body, and soul.

Consultation: A complimentary consultation is available to discuss your needs and determine which service offering best suits you.

Service Packages: Birth Companion: Support during labor and birth for clients with a strong support system. Wholesome Birthkeeping: Comprehensive support during pregnancy, birth, and postpartum for first-time parents or those seeking a dignified birth experience. Postpartum Dieta: Postpartum support focusing on nourishment and recovery after birth.

Payment Terms: Full payment must be made upon booking the service. Payment plans are available and must be agreed upon before booking confirmation.

Refund Policy: All services are non-refundable. In the case of Force Majeure or other extraordinary circumstances, an alternative doula will be provided, or rescheduling options may be explored. If services are canceled by the client, no refunds will be issued.

Client Responsibilities: Clients must communicate any changes in circumstances and ensure they are available for scheduled appointments. Clients are also expected to maintain respectful communication throughout the process. An individual agreement explaining the full extent of rights and responsibilities will be presented to each client before booking services.

Liability: Iveth is a certified full-spectrum doula and is not responsible for medical care, complications, or outcomes. Services provided are non-medical and focus on holistic and emotional support throughout the birth journey.

Service Modifications: Any modifications or additional services must be agreed upon in writing and may incur additional fees.

Privacy and Confidentiality: All personal information and discussions will be kept confidential in compliance with HIPAA regulations.

III. Medicaid Birth Doula Services

1. Introduction: By utilizing the Medicaid Birth Doula Services provided by Iveth Balanta, you agree to the following Terms and Conditions. These terms outline the rights and responsibilities for both the service provider and the client (the birthing individual and family).

2. Services Provided: Iveth Balanta offers Medicaid-covered birth doula services, including emotional, physical, and informational support during pregnancy, labor, delivery, and the postpartum period. Services provided include but are not limited to: Prenatal counseling; Labor support; Postpartum support; Evidence-based information on pregnancy, childbirth, and newborn care; Assistance with infant feeding and breastfeeding guidance; Resources for community services; Emotional support for family and partner.

3. Eligibility and Coverage: These services are covered by Nevada Medicaid and include: A maximum of four (4) 60-minute visits; Support during labor and birth; and Two (2) additional visits only if prescribed by a medical provider. Medicaid benefits are subject to verification. Clients will be required to complete a discovery call, verify Medicaid benefits, and sign an agreement of rights and responsibilities.

4. Scheduling and Availability: Once services are confirmed, clients will schedule appointments via the client portal. Appointments may be scheduled based on availability and the client’s specific needs. Please note that additional services outside the scope of Medicaid coverage may incur out-of-pocket expenses.

5. Fees: Medicaid covers the cost of the services as outlined above. Any services outside of the Medicaid-covered visits (e.g., additional visits, educational materials, or extended support) will be charged separately and are the responsibility of the client.

6. Client Responsibilities: Clients are expected to: - Provide accurate personal and health information. - Follow all necessary steps to verify Medicaid benefits. - Complete required paperwork and agreements. - Respect the service provider’s availability and appointment scheduling.

7. Confidentiality and HIPAA Compliance: All services are provided in compliance with HIPAA (Health Insurance Portability and Accountability Act). Client information will remain confidential, and privacy will be maintained during all interactions.

8. Termination of Services: The service provider reserves the right to terminate services in the event of non-compliance with these terms, non-payment for out-of-pocket services, or any behavior that disrupts the safe and supportive environment required for effective care.

9. Limitation of Liability: Iveth Balanta and Matríz, Territorio y Memoria LLC will not be held liable for any medical complications or outcomes arising during pregnancy, labor, or postpartum. The doula services provided are non-medical in nature and are meant to complement, not replace, medical care.

10. Changes to Terms: These Terms and Conditions are subject to change. Clients will be notified of any updates or modifications. Continued use of services will constitute acceptance of the new terms.

IV. Ancestral Lineage Healing Sessions
  • Practitioners of ancestral lineage healing agree to a code of ethics that can be referenced at: ancestralmedicine.org/code-of-ethics.

  • Ancestral lineage healing is not a substitute for medical or mental health care.

  • Practitioners do not diagnose or treat physical/medical or psychological conditions.

  • Ancestral healing practitioners are not trained to represent any specific religious or spiritual traditions, and the ancestral lineage healing method does not presume to reflect practices of ancestor reverence and ritual from any singular cultural background.

  • This practitioner (Iveth Balanta) is in training and may share session information with Ancestral Medicine supervisors with the intent to receive support and training. When doing so, practitioners honor confidentiality by changing identifying client information.

  • Although practitioners-in-training receive supervision and support from Ancestral Medicine supervisors, they are solely responsible for their work with clients.

  • By signing this disclaimer before acquiring an ancestral Lineage Healing Session, clients confirm that they have had the opportunity to review and agree with the information above, and release Ancestral Medicine from any liability regarding ancestral healing sessions.
